新辅助化疗对胃癌5年生存率影响的Meta分析

目的应用Meta分析的方法探讨术前新辅助化疗对胃癌5年生存率的影响。方法通过Pubmed、Cochrane图书馆、EMBASE、Ovid检索系统、Google搜索引擎及手工检索有关胃癌新辅助化疗的临床对照研究。按纳入排除标准进行筛选,并对入选研究的文献进行质量评价,提取资料进行Meta分析。结果 2000～2007年共8项随机对照试验1 510例胃癌患者纳入分析,其中新辅助化疗后手术治疗组718例,直接手术组792例。Meta分析结果显示新辅助化疗后手术与直接手术相比,死亡风险优势比(OR)值为0.67,95%可信区间为0.53～0.83,P=0.000 3,差异有统计学意义。结论与直接手术相比,手术前行新辅助化疗与提高患者5年生存率有一定相关性。

Meta analysis:The effects of neoadjuvant chemotherapy for 5-year survival rates of the patients with gastric cancer

Objective To evaluate the effects of neoadjuvant chemotherapy for 5-year survival rates of the patients with gastric cancer by Meta-analysis.Methods Search all of clinical randomized trials on neoadjuvant chemotherapy for gastric cancer in MEDLINE,Cochrane Library,EMBASE,Ovid,CBM database and Google.A Meta-analysis was carried out by collecting information based on inclusion and exclusion criteria from all trails′ information available.Results The Meta-analysis included 8 trials.A total of 1 510 patients were included in the analysis,of which 718 patients were in the study group(neoadjuvant chemotherapy plus surgery group) and 792 in the control group(immediate surgery group).According to the Meta analysis to the information available,There was statistical significant diference in 5-year survival rates between the study group and the control group,and the odds ratio(OR) was 0.67,with 95%CI 0.53 to 0.83,P=0.000 3.Conclusion Compared with immediate operation,neoadjuvant chemotherapy followed by operation is in some distance related with increasing 5-year survival rates of patients with gastric cancer.Due to the limited sample and the potential publication bias,the final conclusion still needs further trials to be evidenced.
