低钾性周期性麻痹148例临床分析

摘    要:目的:探讨低钾性周期性麻痹(Hypo PP)的临床特点。方法:回顾性分析14 8例患者的临床和实验室检查资料,观察血钾下降程度、肌力改变、肌酶及心电图变化的相互关系。结果:1血钾1.15 mmol/ L~1.95 mm ol/ L组和2 .0 2 mmol/ L~2 .80 mmol/ L组分别与3.0 4 mmol/ L~3.30 mmol/ L组比较,双下肢肌力减弱,血钠、血氯及血肌酶升高,差异具有统计学意义(P<0 .0 5 )。2心电图改变最常见有U波出现、T波低平、P- R段延长、ST段压低及不同类型的心律失常,以窦性心律失常、房室传导阻滞、室性早搏多见,其改变与血钾下降呈正相关。补钾治疗前后,心电图改变具有统计学意义(P<0 .0 5 )。结论:Hypo PP的肌力、血钾下降程度,肌酶升高程度及心电图改变,有助于判断病情。心电图是一种快速、简便、安全、经济的检查方法,对Hypo PP的诊治及预后具有重要意义。

Abstract:Objective The present study was to observe clinical features of Hypokalemia periodic paralysis.Methods To analyze the data of 148 cases HypoPP in emergence room of my hospital from 2002 to 2004 by retrospective study.Combining the force of muscle, electrolyte,CK and electrocardiogram with serum potassium were used.Results ①The changes of muscle force is closely related with reduced serum potassium in HypoPP. Decreasing of muscle force and increasing of serum sodium and chloride concentration are significant in group 1.15~1.95 and 2.02~2.80 compared with group 3.04~3.30(P<0.05). ②ECG changes were common, U waves ,T waves and ST segment downward or upward and Q-T, P-R period prolongation with arrhythmia were existed. Sinus arrhythmia, atrioventricular block and premature ventricular beat are commonest corresponding with reduced potassium concentration.There were significant difference in ECG before and after potassium treatment(P<0.05).Conclusions The reduced muscle force and potassium and increased CK combining with the changes in ECG can reflect the severity and prognosis of HypoPP. ECG recording is simple, fast and cheap. It is value in diagnose and therapy of HypoPP.
