64层螺旋CT脑动脉瘤血管成像的应用研究

摘    要:目的:评价64层螺旋CT脑动脉成像在脑动脉瘤诊断中的临床应用价值。方法:57例患者全部进行了脑动脉CTA和DSA检查,将DSA检查结果作为参考标准,评估CTA诊断脑动脉瘤的敏感性、特异性、准确性以及阴性预测值。结果:脑动脉CTA49例共有50个动脉瘤,而DSA检查51例发现52个动脉瘤。CTA诊断动脉瘤的特异性为100%,敏感性96.08%,准确性96.5%,阴性预测值75%。结论:64层螺旋CT脑动脉成像诊断动脉瘤特异性、敏感性和准确性高,可以作为脑动脉瘤筛查和术前评估的首选方法,但仍有假阴性,不能完全取代DSA。

Abstract:Objective:To prospectively compare the effectiveness of 64 -slice spiral CT angiography in the detection of cerebral aneurysms. Methods : Fifty-seven patients underwent the inspection by 64 - slice spiral CT angiography and DSA. Using DSA interpretation as the reference standard, the sensitivity, specificity, accuracy, and negative predictive value in diagnosing cerebral aneurysms by CTA were evaluated, respectively. Results:Fifty aneurysms were found in 49 patients CTA, meanwhile, DSA revealed 52 anenrysms in 51 patients. The mean sensitivity, specificity, accuracy and negative predictive value of CTA for the detection of cerebral aneurysm were 96.08 %, 100%, 96.5 % and 75 % , respectively. Conclusions: The 64-slice spiral CT angiography has high sensitivity, specificity, and accuracy for detecting cerebral aneurysm, it could be served as a first choice for screening of cerebral aneurysms and preoperative assessment. However, the CT angiography is currently not sensitive enough to replace digital subtraction angiography.
