胃大部切除术后残胃功能性排空障碍诊治体会

目的探讨胃大部切除后残胃功能性排空障碍的病因、诊断及治疗措施。方法回顾性分析1994~2004-12发生的胃大部切除术后胃功能性排空障碍36例患者的临床资料。结果36例中,9例因误诊为输出机械性梗阻而再次手术。本组无死亡病例,均治愈出院。结论残胃和空肠上段的正常运动功能障碍是主要病因,可能与手术造成支配胃和空肠上段的自主神经损伤有关。非手术治疗是主要的治疗手段,应尽量避免早期再次手术。

Diagnostic and Treatment Experience of Functional Evacuation Disorder of Gastric Remnant after Subtotal Gastrectomy

Objective To discuss the etiology,diagnosis and treatment method of functional evacuation disorder of gastric remnant after subtotal gastrectomy.Methods The clinical material of 36 cases of such patient from 1994 to December 2004 were analyzed retrospectively.Results 9 of 36 cases were re-operated due to the export mechanic obstruction.There was no dead in this group and all healed.Conclusion The main reason was motive dysfunction of gastric remnant and jejunum that might be related with injury of autonomic nerve during operation.The conservative treatment might be main method to avoid re-operation.
