机械通气中的尿量问题

摘    要:目的分析机械通气期间的多尿现象及其相关因素和意义。方法对44例机械通气患进行回顾性分析。结果机械通气期间有65.91%的患出现多尿,多出现于上机后1-3d。多尿症患以慢性阻塞性肺疾病(COPD)合并呼吸衰竭及右心功能不全居多,与使用利尿剂和氨基糖苷类药物也有一定关系。多尿死亡率较低,但尿量显增加、持续时间较长可引起低血压和水、电解质紊乱。结论机械通气期间多尿是较为普遍的现象,应慎用利尿剂和氨基糖苷类药物,及时纠正水、电解质紊乱。

Polyuira during mechanical ventilation
Abstract:Objective To investigate the incidence,significance and contributing factors of polyuria during mechanical ventilation MethodsRetrospective study was performed in 44 subjects receiving mechanical ventilation based on admission rec ords in intensive care unit from Nov 1999 to Jun 2000 Results Polyuria oc curred in 65 91% of subjects being investigated during ventilation,most of whi ch were patients with COPD Polyuria was also associated with utility of diureti cs and aminoglycosides Mortality was lower in subjects with polyuria than that in non-polyuria Excessive urinary output lasting for a relative long time m ay result in hypotension,disturbances of water and electrolytes Conclu sionsPolyuria is not a rare phenomenon during mechanical ventil ation It is essential to use diuretics and aminoglycosides cautiously in order to maintain water and electrolyte homostasis
