532nm绿激光与药物治疗中心性浆液性脉络膜视网膜病变的远期疗效分析

目的分析532nm绿激光治疗中心性浆液性脉络膜视网膜病变（中浆）的远期疗效。方法收集2006年1月~2009年12月末在我院眼科门诊就诊的初发中浆患者90例（90眼）,其中48例（48眼）作为激光治疗组,采用532nm绿激光光凝进行渗漏点直接封闭;单纯药物治疗的42例（42眼）作为对照组。随访12~18个月,观察两组患者复发率和视力变化。结果激光组治疗后12个月和18个月时复发患者分别为2眼（4.2%）和1眼（2.1%）;对照组治疗后12个月和18个月时复发病例分别为15眼（35.7%）和11眼（26.2%）。激光组在治疗后12个月（2=14.55,P〈0.01）和18个月（2=8.49,P〈0.05）时的复发率明显低于对照组。激光组治疗后12个月和18个月时平均视力为（0.95±0.26）和（0.99±0.12）;对照组治疗后12个月和18个月时平均视力为（0.86±0.33）和（0.91±0.28）。但治疗后12个月和18个月时激光组的平均视力明显高于对照组（u=4.31,P〈0.01和v=16.3,P〈0.01）。结论 532nm绿激光治疗中将操作简便且远期疗效确切,少有复发,疗效明显优于单纯药物治疗。

Clinical study on long-term efficacy and safety of treating CSC( central serous chorioretinopathy) with 532nm green laser and medicine

Objective To evaluate the long term results of 532nm green laser photocoagulation on central serous chorioretinopathy.Methods 90 CSC patients（90 eyes） were collected in our hospital from Jan,2006 to the end of Dec,2009.48 cases（48 eyes） were treated by 532nm green laser photocoagulation（laser group）.42 cases（42 eyes） were treated by medicine（drug group）.The patients funds visual acuities were observed and the following-up was made in 12 months to 18 months.Results 2 eyes（4.2%） were recurred until the 12 months and 1 eye（2.1%） until the 18 months of the photocoagulation in laser group;while in drug group,15 eyes（35.7%） were recurred until the 12 months treated and 11 eyes（26.2%） until the 18 months.The effects in laser group was better than that of drug group until the 12 months（χ2=14.55,P0.01） and the 18 months（χ2=8.49,P0.05）of treatment,visual acuities were（0.95±0.26） until the 12 months of photocoagulation and（0.99±0.12） until the 18 months.The visual acuities were（0.95±0.26） until the 12 months treated by medicine and（0.99±0.12） until the 18 months.The visual acuities of laser group were better than that of drug group until the 12 months and the 18 months by treated（u=4.31,P0.01 and v=16.3,P0.01）.Conclusion Treating CSC is quite simply and more available and rare recurring with the 532nm green laser photocoagulation.Its long term effect is much better than medicine.
