结核性腹膜炎并发肠梗阻的诊断及外科治疗

目的探讨结核性腹膜炎并发肠梗阻的诊断及外科治疗方式。方法回顾性分析1990年6月至2008年3月我院经外科治疗的28例结核性腹膜炎并发肠梗阻患者的临床资料。结果本组患者术前确诊12例。全组均行手术治疗，其中右半结肠切除术9例，回盲部切除术5例，小肠部分切除术8例，肠粘连松解术5例，小肠结肠短路手术1例。治愈27例，死亡1例。结论结核性腹膜炎并发肠梗阻的临床表现无特异性，易误诊，密切观察病情变化，选择合适手术时机及术中正确判断手术方式，是手术治疗的关键。

Diagnosis and surgical treatment of tuberculous peritonitis complicated with intestinal obstruction

Objective To investigate the diagnosis and surgical treatment of tuberculous peritonitis complicated with intestinal obstruction. Methods Clinical date of 28 cases of tuberculous peritonitis complicated with intestinal obstruction were retrospectively analyzed from June, 1990 to March,2008. Results Twelve cases were diagnosed before operation. Patients received different surgical procedures as following ： right colectomy in 9 patients, ceeectomy in 5 patients, partial enterectomy in 8 patients, lysis of adhesion in 5 cases, ileocolic bypass procedure in 8 cases. Twenty - seven cases were cured and one case was died. Conclusions It is easy to make a mistake in diagnosis because of lacking of characteristic clinical manifestations. It＇ s important to investigate carefully patient＇ s condition and select the operative opportunity and method.
