摘    要:目的探讨肺结核短程化疗方案中链霉素静脉滴注与肌肉注射两种不同用药途径对肺结核治疗效果及不良反应的影响。方法对289例初治菌阳的住院肺结核病人,按入院顺序随机分为链霉素静脉滴注组与肌肉注射组进行观察。结果静脉滴注组病人痰菌阴转平均天数(29±18)d短于肌肉注射组(33±22)d、胸部X线病灶吸收及空洞闭合疗效构成比静脉滴注组高于肌肉注射组,但以上情况两组对比差异均无统计学意义(P〉0.05)。肝肾功能损害及耳毒性两组对比差异无统计学意义(P〉0.05)。病人对所采用注射方法的满意度静脉滴注组明显高于肌肉注射组,两组对比差异有统计学意义(U=2.09,P〈0.05)。结论链霉素静脉滴注,可有效减轻病人因肌肉注射引起的疼痛和局部不良反应,提高痰菌阴转率,缩短了空洞闭合时间,毒副作用未见增加,为本药的应用提供了新的用药途径。

Therapeutic effect for tuberculosis with positive sputum treated by intravenous streptomycin in short - course chemotherapy

Abstract:Objective To explore influence of effect and side - effects for tuberculosis we took two different ways of using intravenous streptomycin drop and intramuscular injection in PTB short - course chemotherapy. Methods 289 cases of inpatients with PTB showing positive sputum were divided into two groups according to the order admitted into the hospital and randomly, one treated with streptomycin intravenous drop and the other with intramuscular. Results The average days of sputum negative conversion in intravenous drop group (28 ± 18 )d were shorter than in intramuscular group (33 ±22) , and the ratio of X - ray nidus absorption and cavity occlusion in intravenous drop group was higher than in intramuscular group, but the above mentioned, there were no remarkable difference between two groups (P 〉 0.05). There were no remarkable difference in hepat -kidney function lesion ( P 〉 0.05), and there were no any difference about oto - toxicity on two groups ( P 〉 0.05). The patients in intravenous drop group are more content than in intramuscular group, and there was remarkable difference in two groups ( u = 2.09 P 〈 0.05). Conclusions Intravenous streptomycin can deliver the patients' pain and local side - effects resulting from injection, increase the sputum negative conversion, shorten the time of cavity occlusion. Toxico - side effect has no been found increasing. So the application of this medicine supplies is a new way for it.
