长托宁联合阿托品治疗急性重度有机磷中毒

摘    要:目的观察新型抗胆碱药物长托宁联合阿托品治疗急性重度有机磷农药中毒(SAOPP)的疗效。方法随机将SAOPP患者120例分为长托宁治疗组、长托宁+阿托品治疗组及阿托品治疗组三组均结合氯解磷定治疗。观察患者用药次数、阿托品化时间、症状消失时间、全血乙酰胆碱酯酶(ChE)活性恢复时间、治愈时间、治愈率和病死率。结果长托宁阿托品组在用药次数、阿托品化时间、症状消失时间、全血ChE活性恢复时间、治愈时间、治愈率、病死率与其他两组比较差异有统计学意义(P〈0.05),长托宁组与阿托品组比较差异有统计学意义(P〈0.05)。结论长托宁加阿托品治疗AOPP起效快、简便易行、安全可靠,值得推广应用。

Abstract:Objective To investigate the therapeutic effect of penehyclidine hydrochloride and atropine on severe acute organophosphorus pesticide poisoning (AOPP). Methods One hundred and twenty SAOPP patients were randomly divided into three groups: penehyclidine hydrochloride and atropine group, penehyclidine hydrochloride group andatropine group. Patients in three groups were treated with gastrolavage, pralidoxime chloride, penehyclidine hydrochloride and atropine treatment. The pene-hyclidine hydrochloride and atropine dosage, time of disappearance of symptoms, the recovery time of acetyl cholinesterase (AChE) , atropinization time, hospitalization days, cure rate, mortality were also observed and to evaluate the clinical effects. Results The penehyclidine hydrochloride and atropine dosage, time of disappearance of symptoms, the recovery time of acetyl cholinesterase ( ACHE), atropinization time, hospitalization days, curerate, fatality were significantly better than other groups ( P 〈 0.05 or P 〈0.01 ) in terms of time of disappearance of symptoms, the recovery time of acetyl cholinesterase (ACHE) , atropinization time, hospitalization days. The terms of penehyclidine hydroehloride group were better than atropine group. Conclusions The pre -hospital systematic treatment can improve the prognosis of the patients with SAOPP, which is worth popularizing penehyclidine hydrochloride and atropine treatment for SAOPP patients should he used and recommended as a safe and effective treatment strategy.
