溃疡性结肠炎的内镜分期、分型与临床表现的关系

目的　分析溃疡性结肠炎(UC)的内镜分期、分型与临床表现的关系。方法　收集经结肠镜和病理、临床表现进行排除性诊断而确诊的UC 1 78例,对其临床表现、内镜下炎症分期、炎症累及范围及性别之间的相互关系进行分析。结果　UC男性多见。发病年龄30～5 9岁占2 /3以上,临床表现以腹痛、腹泻和便血常见。病变累及左侧结肠以下(72 . 6 %) ,累及次全和全结肠者(1 9 . 1 %) ;A1 、A3 期以便血为主,A2 期以腹痛为主,缓解期以腹泻、腹痛为表现。发热常出现在A期和CA期;性别的差异与炎症程度无关,而Ⅲ和Ⅴ型同一累及范围男性较女性明显增加(P <0 . 0 1 )。结论　溃疡性结肠炎在临床上主要表现为腹泻、腹痛、便血、发热,但不同类型临床表现各异,与内镜下炎症分期、累及范围及性别之间有密切关联。

ENDOSCOPIC AND CLINICAL FEATURES OF 178 PATIENTS WITH ULCERATIVE COLITIS

Objective To analyze the endoscopic models, stages and its correlation with clinical expression in the patients with active period of ulcerative colitis (UC). Methods The 178 cases of UC were collected according to the diagnostic criteria of Chengdu conference. The data were analyzed for sex, clinical feature, the lesion range and the course of illness by endoscope. Results The male were more much than female in the proportion of patients diagnosed as UC. The age from 30 to 59 was exceeded two thirds. The typical clinical displays of UC were bloody stool, diarrhea and abdominal pain. Left-side colitis or proctitis were found in 72.6%, pancoltistis or subpancoltistis in 19.1% respectively. Conclusions The types of clinical feature were in relation with the lesion range, the course of illness by colonoscopy and sex.
