胰腺囊性肿瘤的诊断和治疗

目的 探计胰腺囊性肿瘤的诊断和治疗。方法 回顾性分析22例胰腺囊性肿瘤的临床资料。男8例。女14例。14例（63.3％）为胰腺囊腺瘤（浆液性6例、黏液性8例），8例（36．3％）为胰腺囊腺癌。两者之比为1．7：1。肿瘤位于胰腺头部6例（27．2％），胰腺体尾部12例（54．5％），胰腺尾部4例（18．1％）。行胰腺肿瘤局部切除术1例。胰腺体尾部切除术14例，胰头十二指肠切除术3例，胰腺中段切除术2例。囊肿-空肠Roux-Y吻合术1例，T管架桥胆-肠内引流术1例。结果 随访10～15年，8例囊腺瘤切除术后全部无瘤生存。囊腺癌切除术后1年存活率为100％，3年存活率为87．5％，5年存活率为50％。结论 只要患者无手术禁忌。应主张手术切除为妥。胰腺囊腺癌的恶性程度较低。根治性切除术预后良好。

Diagnosis and treatment of pancreatic cystic tumor

Objective To explore the diagnosis and treatment of pancreatic cystic tumor. Methods The clinical data of pancreatic cystic tumor were analyzed respectively in 22 cases. They were male in 8 cases and female in 14. Pancreatic cystoadenoma was in 14 cases （63.3%） and pancreatic cystoadenocarcinoma in 8（36.3%）. It was 1.7：1 between two groups. Tumor at the head of pancreas was 6 cases （27.2%）, at body-tail 12 （54.5%）, at tail 4（18.1%）. Local resection was performed in one case, pancreatic body-tailectomy in 14, pancreatoduodenectomy in 3, central pancreatectomy in 2 cases. Cystojejunostomy was performed in 1 and choledochojejunostomy bridge through “T” tube in 1 case. Results Follow-up for 10-15 years, all 8 pancreatic cystoadenomas are alive, post-cystoadenocarcinomectomy survival for 1 year is 100%, for 3 years 87.5%, for five years 50%. Conclusion Pancreatic cystic adenocarcinoma is low malignant potential tumors, radical resection of tumors can obtain good prognosis.
