瓣膜病术后房性心动过速的射频消融治疗

目的应用电解剖标测系统分析瓣膜病换瓣术后房性心动过速的电生理机制及射频消融治疗的疗效。方法瓣膜病换瓣术后房速患者共11例，其中男4例，女7例，年龄25～65岁，房速历史4月～6．5年。电解剖标测系统完成心房电压和激动标测，分析心动过速的机制并确定消融靶点，使用冷生理盐水灌注导管消融。结果11例患者中10例患者消融成功，1例患者为左房起源房速，放弃消融。10例消融患者中，共诱发出13种房性心动过速，其中局灶性房速3种，微小折返性房速2种，大折返性房速8种。消融后，房速均不能诱发。术后1例复发，经再次消融成功后无复发。结论瓣膜病换瓣术后房速运用电解剖标测导航下的射频消融有良好效果。

Radiofrequence ablation of atrial tachycardia following valvular surgery

Objective To evaluate the therapeutic effects of atrial tachycardia（AT） after valvular surgery using catheter radiofrequency ablation（RFCA） with Carto system mapping. Methods Eleven patients with AT after valvular surgery, 4 males and 7 females, aging from 25 to 65 years, were enrolled in this study. Under the guidance of Carto system mapping,RFCA was applied. Results RFCA was given up in one patient with left atrium tachycardia. Other 10 patients underwent RFCA and there were 13 AT induced, including 3 focal AT, 2 micro reentry AT,and 8 macro reentry AT. Successful ablation was accomplished in these 10 patients and no procedural complications. One patient was seen recurrent AT and accepted another successful ablation. Conclusion RFCA with Carto system mapping is a safe and effective method for treating AT after valvular surgery.
