兔耳纯静脉皮瓣成活机理的实验研究

摘    要:目的:探讨纯静脉皮瓣的成活机理。方法:①利用兔耳纯静脉皮瓣模型,观测不同处理后的成活情况;②活体灌注墨汁后厚蜡切片,观察皮瓣术后与受区的血管吻合情况。结果:原位缝合、边缘阻隔、基底阻隔、全部阻隔、术后4d断蒂纯静脉皮瓣成活面积大于术后3d断蒂纯静脉皮瓣和无血供全厚皮片(P<0.05)。结论:静脉血可以维持纯静脉皮瓣成活,术后3~4 d是纯静脉皮瓣血液循环由非生理性向生理性转变的关键。

Abstract:Objective: To research the survival mechanism of the pure venous flap. Methods: (1) Pure venous flaps in rabbit ears were treated with different ways and their survival state was observed respectively; (2) After ink perfusion in vivo, the flaps were embedded in paraffin wax and sliced, and the post-operative vascular anastomasis was observed between the flap and the recipient site. Results: The pure venous flaps of replacement suture, margin isolation, base isolation, overall isolation and the 4th-day pedicle cutting had larger survival surfaces than those of the 3rd-day pedicle cutting and those full-thickness ones without blood supply (P<0.05). Conclusions: The venous blood can maintain the survival of the pure venous flap and the post-operative period between the 3rd and the 4th day is crucial to the transition from non-physiological to physiological blood circulation of the pure venous flap.
