降纤酶对缺血性脑卒中急性期血浆VEGF等4项生物分子的影响

摘    要:目的:观察降纤酶治疗早期缺血性脑卒中前后血浆中血管内皮生长因子(vascular endothelial growth factor VEGF)、内皮素(endothelin ET-1)、P-选择素(P-selectin)和血小板活化因子(platelet activating factor PAF)水平的变化,并探讨其机理。方法:随机将114例急性缺血性脑卒中患者分为降纤酶组和对照组,观察其临床疗效,检测凝血功能和VEGF、ET-1、P-seleetin、PAF等4项生物分子水平的变化。结果:治疗后2组临床疗效经Ridit分析差异无统计学意义(P〉0.05),2种药物对凝血功能和4项血浆生物分子水平的影响差异有统计学意义(P〈0.01),降纤酶抗凝明显,且能降低Fbg水平,对ET-1、P-seleetin和PAF有抑制作用,能提高VEGF的血浆含量。低分子肝素钠抗凝明显,也能降低Fbg水平,对PAF有抑制作用。结论:降纤酶可提高血浆VEGF水平,抑制血浆ET-1、P-selectin和PAF的合成。

Abstract:Objective: To explore the changes of plasma vascular endothelial growth factor(VEGF),endothelin(ET-1),P-selectin and platelet activating factor(PAF)in patient with ischemic stroke treated with defibrinogenase.Method: The blood coagulation function and the concentration of VEGF,ET-1,P-selectin and PAF were determined in acute cerebral infarction in 66 patients with defibrinogenase and 48 with low dose heparin before and after treatment.Result: Excluding curative effect,there was significant difference between the two groups on the blood biochemicals items and plasm biomoleculs levels.The Fbg decreased,the level of ET-1,P-selectin and PAF were depressed,and the plasm VEGF level rised in the defibrinogenase group,whereas only Fbg and the plasm PAF were depressed by low dose heparin.Conclusion: Defibrinogenase seems to have a salutary effect on descending of the Fbg,rising of the plasm VEGF levels and depressing of the synthesis of the ET-1,P-selectin,and PAF.
