血浆脑钠肽测定在评估慢性肾功能不全患者心功能中的价值

目的探讨血浆脑钠肽(BNP)测定在评估慢性肾功能不全(CRF)患者心功能中的应用价值。方法测定283例CRF患者的血浆BNP,比较不同肾功能级别、不同心功能级别、心肾功能综合分级状态下患者血浆BNP水平变化及其诊断心功能的价值。结果随着肾功能分期及心功能分级的增高(分期越高,肾功能越差;分级越高,心功能越差),患者血浆BNP水平越高(P<0.01)。在同一CRF级别中,血浆BNP水平随着心功能的恶化逐渐升高(P<0.01);在同一美国纽约心脏病学会的心功能分级中,随着肾功能不全的进一步加重,血浆BNP水平逐渐升高(P<0.01)。受试者工作特征曲线分析,当选160 ng.L-1为诊断界点时,灵敏度为84.2%,特异度为81.7%,准确率为85.5%;选180 ng.L-1为诊断界点时,灵敏度为82.7%,特异度为84.6%,准确率为83.3%;选200 ng.L-1为诊断界点时,灵敏度为80.1%,特异度为87.8%,准确率为82.2%。结论血浆BNP测定有助于评估CRF患者心功能。
