开腹与腹腔镜辅助治疗小肠间质瘤的临床对比分析

目的：探讨腹腔镜手术治疗小肠间质瘤的有效性、安全性。方法：回顾分析2008年1月至2012年12月收治的65例小肠间质瘤患者的临床资料。其中35例行腹腔镜手术（腹腔镜组），30例行开腹手术（开腹组），观察两组患者一般资料、临床症状与体征及术后随访资料等。对比两组患者术后恢复排气时间、住院时间及并发症发生率。结果：两组在性别构成、年龄大小、临床症状构成、肿瘤最大径、切除方式、术后并发症等方面差异无统计学意义（P〉0．05）。腹腔镜组手术时间、术后恢复排气时间、术后住院时间显著优于开腹组（P〈0．05）。结论：术者具有丰富的胃肠道手术及腹腔镜手术经验，为最大径〈5cm的小肠间质瘤患者行腹腔镜手术是安全、有效的，术中防止肿瘤破裂尤为重要。

Cfinically comparative analysis of laparotomy and laparoscopic-assisted treatment for small intestine stromal tumors

Objective：To evaluate the effectiveness and safety of laparoscopic operation for stromal tumor of small intestine. Methods ：The clinical data of 65 patients with stromal tumor of small intestine from Jan. 2008 to Dec. 2012 were retrospectively analyzed. They were divided into 2 groups ： laparoscopy group （ n = 35 ） and open group （ n = 30）. The basic information, clinical symptoms and signs and follow-up data were observed. The postoperative exhaust time, hospital stay and the incidences of complications were compared between the two groups. Results：There were no significant differences in the gender composition, age, clinical symptoms, the maximum diameter of tumor, way of resection, postoperative complications between two groups （ P 〉 0.05 ）. However, the operative time, postoperative exhaust time and hospital stay in the laparoscopy group were significantly less than those in the open group （P 〈 0.05 ）. Conclusions ： For surgeons with enough experience both in the gastrointestinal operation and laparoscopic surgery, laparoscopic resection of the small intestine stromal tumors （maximum diameter 〈 5 cm） is safe and effective, and it is especially important to avoid the rupture of tumors.
