XX医院重型肝炎患者肠球菌感染的现状及药敏分析

目的： XX医院重型肝炎患者肠球菌感染的现状及药敏分析。方法收集在该院住院治疗的重型肝炎患者244例，比较急性重性肝炎、亚急性重性肝炎、慢性重性肝炎中粪肠球菌、屎肠球菌、鸟肠球菌感染率的差异；分析各型肠球菌对13种抗生素的敏感性；比较单纯肠球菌感染和混合肠球菌感染患者的生存时间。结果244例研究对象中粪肠球菌总感染率（179，73．36％），屎肠球菌总感染率（52，21．31％），鸟肠球菌总感染率（13，5．33％）；各类型重性肝炎患者中不同肠球菌感染构成的差异具有统计学意义（ P＜0．05）。万古霉素和替考拉宁对各型肠球菌的敏感性较高，总的敏感率分别为95．90％和97．54％；头孢唑林、头孢曲松、氯霉素和红霉素的敏感率最低，均在30％以下；粪肠球菌、屎肠球菌、鸟肠球菌对大多数抗生素（除万古霉素和替考拉宁）的敏感性差异具有统计学意义（ P＜0．05）。单纯肠球菌感染的患者5年随访中位生存期24月显著的高于混合肠球菌感染患者的19个月（log‐rank χ2＝106．329，P＜0．001）。结论万古霉素和替考拉宁对各型肠球菌的敏感性较高，单纯肠球菌感染的患者的生存期高于混合肠球菌感染患者。

Current situation of the infection of the patients with severe hepatitis in XX hospital and its drug sensitivity analysis

Objective To investigate the current situation of the infection of the patients with severe hepatitis in XX hospital and its drug sensitivity analysis .Methods 244 Cases of severe hepatitis patients who were hospitalized in our hospital ,to compare differences in the infection rate of enterococcus faecalis 、enterococcus faecium、enterococcus avium in acute severe hepatitis 、subacute hepatitis and chronic hepatitis ;Analysis of the sensitivity of the 13 kinds of antibiotics to the differences type of enterococcus ;To compare the survival time of the patients with simple and mixed infection of the enterococcus .Results The total infection rate of enterococcus faecalis(179 ,73 .36% ) ,the total infection rate of enterococcus faecium (52 ,21 .31% )and the total infection rate of en‐terococcus avium (13 ,5 .33% )in 244 cases .There were significant differences in infections rate of enterococcus in different types of hepatitis patients(P<0 .05) .High sensitivity to vancomycin(95 .90% )and teicoplanin(97 .54% )for each type of enterococcus ;The sensitive rate of Cefazolin ,ceftriaxone ,chloramphenicol and erythromycin were below 30% ;Enterococcus faecalis ,Enterococcus faecium ,Enterococcus avium sensitivity for the most antibiotics (except vancomycin and teicoplanin ) ,and the difference was statisti‐cally significant(P<0 .05) .The median survival of the patients with simple infection of 19 months ,which was significantly higher than that of the patients with mixed infection of 24 months .Conclusion High sensitivity to vancomycin and teicoplanin on various types of Enterococcus ,the survival period of patients with simple infection was higher than that of patients with mixed infection .
