3项指标联合检测在原发性肝癌诊断中的临床价值

目的探讨血清异常凝血酶原(DCP)、高尔基体蛋白73(GP73)及甲胎蛋白异质体(AFP-L3)联合检测在诊断原发性肝癌(PHC)中的临床价值。方法选取2013年7月至2015年8月该院肿瘤科收治的78例PHC患者,慢性乙型肝炎(CHB)患者86例,肝硬化(LC)患者70例作为研究对象,另选取同期健康体检者80例作为健康对照组,比较4组研究对象DCP、GP73及AFP-L3在不同人群中的血清水平及各项指标单项及联合检测对PHC的诊断效果。结果 PHC组患者DCP、GPF73及AFPL3水平均显著高于其他组,差异均有统计学意义(P0.05)。对于PHC的诊断,单项检测血清GP73的灵敏度显著高于DCP和AFP-L3,差异有统计学意义(P0.05)。血清DCP、GP73和AFP-L3 3项联合检测的灵敏度、特异度及准确度均显著高于单项检测结果,差异均有统计学意义(P0.05)。结论血清DCP、GP73及AFP-L3联合检测能有效提高PHC的检出率,对PHC有重要临床诊断意义。

The diagnostic value of combined detection of DCP,GP73 and AFP-L3 in primary liver cancer

Objective To investigate the clinical value of combined detection of serum abnormal prothrombin (DCP) ,Golgi pro‐tein 73 (GP73) and AFP‐L3‐in the diagnosis of primary liver cancer .Methods Seventy‐eight patients with primary liver cancer (PHC) ,86 patients with chronic hepatitis B (CHB) and 70 patients with liver cirrhosis (LC) were enrolled in this study from July 2013 to August 2015 ,the other 80 healthy subjects were selected as the control group .The serum levels of DCP ,GP73 and AFP‐L3 in the four groups were compared and the diagnostic efficacy of the single and combined detection of PHC .Results The levels of DCP ,GPF73 and AFP‐L3 in PHC group were significantly higher than those in other groups (P< 0 .05) .For the diagnosis of PHC ,the sensitivity of serum GP73 alone was significantly higher than that of DCP and AFP‐L3 ,the difference was statistically significant (P<0 .05) .The sensitivity ,specificity and accuracy of combined detection of serum DCP ,GP73 and AFP‐L3 were signif‐icantly higher than those of single test (P<0 .05) .Conclusion The combined detection of serum DCP ,GP73 and AFP‐L3 can ef‐fectively improve the detection rate of PHC ,which has important clinical significance for the diagnosis of PHC .
