血清肽素水平对非勺型高血压患者的早期诊断价值

目的血清肽素水平对非勺型高血压患者的早期诊断学价值研究。方法 2014年1月至2015年12月于该院就诊的90例高血压患者,根据24h动态血压监测结果,分为勺型高血压组(n=34)和非勺型高血压组(n=56);另选择50例健康志愿者为对照组。通过相关性分析和受试者工作特征(ROC)曲线等分析比较肽素和非勺型高血压之间的关系。结果非勺型高血压患者白天及夜间的平均收缩压(dSBP、nSBP)、平均舒张压(dDBP、nDBP)均明显高于勺型高血压组患者,差异有统计学意义(P0.05)。而非勺型高血压患者血清肽素水平明显高于勺型高血压患者,差异有统计学意义(P0.05)。ROC曲线分析提示血清肽素水平诊断非勺型高血压的ROC曲线下面积(AUC)为0.868,95%置信区间(CI)为0.812~0.933,cut off值为2.41IU/mL,灵敏度为80%,特异度为86%。相关分析显示血清肽素水平与非勺型高血压呈正相关(r=0.667,P0.05)。Logistic回归分析肽素(是否大于2.41IU/mL)为非勺型高血压发生的独立危险因素。结论非勺型高血压患者血清肽素水平较勺型高血压患者明显升高,肽素是非勺型高血压潜在的诊断学标志物。

The early diagnosis value of serum copeptin for non-dipper hypertension

Objective To evaluate the early diagnosis value of serum copeptin for non-dipper hypertension .Methods Ninety hy-pertension patients were enrolled in this study and divided into two group based on ambulatory blood pressure monitory ,34 patients in dipper hypertension group ,56 patients in non-dipper hypertension group .Correlation analysis and receiver operating characteristic (ROC)curve analysis were used to evaluate the relationship between copeptin and non-dipper hypertension .Results The levels of mean day and night SBP and DBP(dSBP ,nSBP ,dDBP ,nDBP)in non-dipper hypertension group were significantly higher than those in dipper hypertension group(P< 0 .05) .The contents of copeptin in non-dipper hypertension group were significantly higher than those in dipper hypertension group(P< 0 .05) .ROC curve analysis showed that the AUC of copeptin was 0 .868 ,and cut-off value was 2 .41 IU /mL with 80% sensitivity and 86% specificity .The correlation analysis showed that serun copeptin was positively as-sociated with non-dipper hypertension(r= 0 .667 ,P< 0 .05) .Logistic regression showed that copeptin was an independent risk for non-dipper hypertension .Conclusion The levels of copeptin significantly elevates in non-dipper hypertension patients compared with dipper hypertension ,which might be a novel and potential diagnosis biomarker .
