慢性粒细胞性白血病骨髓中嗜酸粒细胞和嗜碱粒细胞的观察

摘    要:目的 探讨骨髓中嗜酸粒细胞、嗜碱粒细胞在诊断慢性粒细胞白血病中所起的作用.方法 对70例慢性粒细胞性白血病按现在国内通用的诊断标准进行复核和确认[1],进行回顾性分析,观察其骨髓象中嗜酸粒细胞、嗜碱粒细胞的情况.结果 嗜酸粒细胞中晚幼阶段与正常参考值比较显著增高,其U值为4.8,P<0.05,杆状核、分叶核阶段与其正常参考值比较,U值为8.5,P<0.05.嗜碱粒细胞百分率的均值与正常参考值比较,显著增高(U=2.3,P<0.05),不能划分阶段的嗜碱粒细胞占大多数,故难于判断其成熟障碍.嗜酸、嗜碱粒细胞形态学上在慢性粒细胞性白血病骨髓中各自有其特点.结论 在慢性粒细胞性白血病骨髓中,嗜酸、嗜碱粒细胞数量上的变化,形态学上的一些特点、对诊断慢性粒细胞性白血病有辅助诊断价值.

Abstract:Objective To investigate functions of eosinophil cells and basophil cells in the bone marrow when diagnosing the chronic myelocytic leukemia(CML) .Methods Firstly,to check and confirm 70 cases of the chronic myelocytic leukemia(CML) ,according to the domestic current diagnostic criterions.Secondly,to observe eosinophil cells and basophil cells in the marrow smear by analysis of data.Results In the immature and mature stages,there is a remarkable increase of eosinophil cells whose"U"value is 4.8,P<0.05,compared with the normal reference value,And in the stage of stab granulocytes and segmented granulocytes,compared with the normal reference value,the "U" value of eosinophil cells is 8.5,P<0.05.Compared with the normal reference value,there is a significant rise of the mean percentage of basophil cells(U=2.3,P<0.05) .Most basophil cells can not be divided into stages.Therefore,it is difficult to judge their mature obstacles.From the view of the morphology,eosinophil cells and basophil cells have respective features ian the bone marrow with the chronic myelocytic leukemia(CML) .Conclusions In the bone marrow with the chronic myelocytic leukemia(CML) ,the change of the number of eosinophil cells and basophil cells,and some features in the morphology have auxiliary diagnostic value in diagnosing the chronic myelocytic leukemia(CML) .
