230例肝硬化患者上消化道出血与门脉高压性胃病关系临床分析

目的：观察肝硬化患者上消化道出血与门脉高压性胃病（PHG）的关系、发生率、表现形式及其内镜特点。方法：收集分析1985年1月－2000年12月间230例肝硬化合并上消化道出血患者的病历资料，所有患者均做急诊胃镜。结果：所有患者均有食管胃底静脉曲线，230例肝硬化合并上消化道出血的原因：曲线静脉破裂出血占38．26％，PHG出血占30．43％，消化性溃疡出血占29．57％，其它占1．74％。PHG的内镜主要表现为各种形态的充血和糜烂。结论：肝硬化患者合并上消化道出血病因中，PHG是重要原因，非食管静脉曲线出血约占60％。

Clinic analysis of the relationship between hemorrhage of upper digestive tract in 230 cirrhotic patients with portal hypertensive gastropathy

Objective:To investigate the relationship between hemorrhage of upper digestive tract in 230 cirrhotic patients with portal hypertensive gastropathy,and incidence,endoscopic appearance.Methods:230 cirrhotic patients with upper digestive tract bleeding from January 1985 to December 2000 which were emergency endoscopied in China Japan Friendship Hospital were studied.Results:All of patients were esophageal varices and/or gastric varices.We analyzed the causes of upper gastrointestinal bleeding in 230 patients,among which ruptured varices,portal hypertensive gastropathy,peptic ulcer were 38.26%,30.43%,29.57%,respectively,and others were 1.74%.Endoscopic appearances of portal hypertensive gastropathy were all forms of congestion and erosion.Conclusion:Among the causes of upper gastrointestinal bleeding in cirrhotic patients,portal hypertensive gastropathy were often observed and non variceal bleeding was about 60%.
