肾囊内注射甲基强的松龙与常规激素治疗原发性肾病综合征的疗效比较

【目的】观察肾脂肪囊（肾囊）内注射甲基强的松龙（MP）及常规激素治疗原发性肾病综合征（PNS）的临床效果及副作用。【方法】诊断为PNS的114例患者，分为常规组（A组）和肾囊内注射组（B组）同时予常规的抗凝及环酸酰胺冲击治疗，观察治疗前及治疗2、8周后尿蛋白、血清白蛋白、血压、血糖、皮下脂肪厚度等指标的变化。【结果】两组在治疗2、8周后尿蛋白减少、血清白蛋白升高，然而两组之间的尿蛋白、白蛋白及血压相比较没有差异。而血糖及皮下脂肪厚度的变化在B组的发生较A组明显降低或减少。【结论】肾囊内注射MP治疗PNS，与传统的给药方式相比，疗效相当，但起效更快且基本无激素副作用。

Renal Intra-capsule Injection of Methylprednisolone in Comparison with Routine Prednisone for Treating Primary Nephrotic Syndrome

Objective]To study and compare the therapeutic effects and side reactions of methylprednisolone injected into renal capsule and routine prednisone for treating patients with primary nephrotic syndrome. Methods]A total of 114 patients diagnosed as primary nephrotic syndrome by renal biopsies were divided into group A （routine prednisone treatment） and group B（intra-capsule injection of methylprednisolone）. All of them were given CTX and other anticoagulation. The urinary protein, plasma albumin, blood pressure and blood glucose, depth of subcutaneous fat before and after the treatment were observed. Results] There was the decrease in urinary protein and improvement in albumin after 8 weeks. However there were no differences between two groups in urinary protein, albumin and blood pressure. The change of depth of subcutane- ous fat and blood glucose in group B was decreased more than that of group A. Conclusion] Renal intra-capsule injection with methyprednisolone is an effective procedure to treat primary nephrotic syndrome, and it has less side effects.
