补肾活血汤联合玻璃酸钠治疗膝关节骨性关节炎60例临床研究

目的:观察补肾活血汤联合玻璃酸钠治疗膝关节骨性关节炎(KOA)的疗效及对血清中Ⅰ型胶原C端肽(CTX-Ⅰ和Ⅱ型胶原C端肽(CTX-Ⅱ)水平的影响。方法:将120例肾虚血瘀型KOA患者采用随机数字表法分为对照组和观察组各60例。2组均采用玻璃酸钠关节腔内注射,并同期进行功能锻炼。观察组加用补肾活血汤内服。2组均连续治疗4周。治疗前后采用西安大略和麦马斯特大学(WOMAC)骨关节炎指数可视化量表评价膝关节功能,以疼痛视觉模拟评分法(VAS)评定膝关节疼痛程度。检测2组治疗前后的血清CTX-Ⅰ和CTX-Ⅱ水平。结果:治疗后,观察组临床疗效优于对照组,差异有统计学意义(P <0.05)。2组WOMAC量表疼痛、僵硬、关节功能评分和总分,VAS评分均较治疗前下降(P <0.01),观察组各项评分低于对照组(P <0.01)。2组血清CTX-Ⅰ和CTX-Ⅱ水平均较治疗前下降(P <0.01),观察组血清CTX-Ⅰ和CTX-Ⅱ水平均低于对照组(P <0.01)。结论:补肾活血汤联合关节腔内注射玻璃酸钠能减轻KOA患者膝关节的疼痛程度,改善膝关节功能,临床疗效显著,调节患...

Clinical Study on 60 Cases of Bushen Huoxue Tang Combined with Sodium Hyaluronate for Knee Osteoarthritis

Objective:To observe the clinical effect of Bushen Huoxue tang combined with sodium hyaluronate for knee osteoarthritis(KOA), and its effect on the levels of C-telopeptide of type Ⅰ collagen(CTX-Ⅰ) and C-telopeptide of type Ⅱ collagen(CTX-Ⅱ) in serum. Methods:Divided 120 cases of KOA patients with kidney deficiency and blood stasis syndrome into the control group and the observation group randomly,60 cases in each group. Both groups received sodium hyaluronate via injection and underwent functional exercise in the same period,while the observation group additionally received Bushen Huoxue tang via oral administration. The treatment of both groups lasted for 4 weeks. Before and after treatment,evaluated knee joint function via the Western Ontario and McMaster Universities(WOMAC) Osteoarthritis Ⅰndex,assessed the degree of pain via Visual Analogue Scale(VAS),and detected the levels of CTX-Ⅰ and CTX-Ⅱ in serum in both groups. Results:After treatment,the clinical effect in the observation group was better than that in the control group,difference being significant(P < 0.05). The scores of pain,stiffness and joint function and the total score in WOMAC and the scores of VAS of patients in both groups were respectively lower than those before treatment(P < 0.01);each score above in the observation group was lower than that in the control group(P < 0.01). The levels of CTX-Ⅰ and CTX-Ⅱ in serum in both groups were lower than those before treatment(P < 0.01),the levels of CTX-Ⅰ and CTX-Ⅱ in serum in the observation group were lower than those in the control group. Conclusion:The application of Bushen Huoxue tang combined with injection of sodium hyaluronate in articular cavity can improve the knee joint function of patients with knee osteoarthritis,alleviate pain and promote clinical effect. One of its functional approaches may be regulating such bone metabolic markers as CTX-Ⅰ and CTX-Ⅱ.
