交锁髓内钉治疗胫骨骨折

摘    要:目的 探讨交锁髓内钉治疗胫骨骨折中的治疗效果。方法 对66例胫骨骨折采用交锁髓内钉内固定治疗,闭合性骨折47例,开放性骨折19例。动力型固定16例,静力型变为动力型固定50例。结果 66例随访10~30个月,平均18个月。骨折均愈合,平均愈合时间16周。术后迟发性感染1例,锁钉偏孔7个孔,大隐静脉损伤1例。根据Johner—Wruh评分标准:优47例,良14例,可差1例,总优良率92.43%。结论 交锁髓内钉内固定术是治疗胫骨骨折的一种理想方法,具有创伤小、固定可靠、应力遮挡小、骨折愈合率高、能早期活动等优点。

Abstract:Objective To explore the clinical effect of interlocking intramedullary nail in the treatment of tibial fracture. Methods 66 cases of tibial fracture were treated with interlocking intramedullary nail. There were 47 closed fractures, 19 opened fractures. 16 cases were treated with dynamic fixation, while 50 cases were changed from static to dynamic fixation. Results All cases were followed up for 10 to 30 months (average 18 mouths) and evaluated according to the criteria of Johner and Wruh. 47 cases were assessed as excellent, 14 as good, 4 as fair and 1 as poor.The excellent and good rate was 92 43%. All fractures healed with the averaged time of fracture union as 16 weeks. Delayed infection happened in 1 case, failure of interlocking in 7 screws, great saphenous vein injury in 1 case. Conclusion The interlocking intramedullary nailing in the treatment of tibial fracture has advantages of less trauma, strong fixation, high healing rate, and early rehabilitation, so it's a good internal fixation to treat tibial fracture.
