多靶点毁损术治疗伴攻击行为精神分裂症

目的探讨CT引导立体定向多靶点联合毁损术治疗伴有冲动攻击行为的难治性精神分裂症的最佳手术方式、临床治疗效果,安全性及其适应证。方法对147例伴有冲动攻击行为的难治性精神分裂症病人行立体定向多靶点联合毁损术,其中138例获1～3年随访,根据CGPS评分标准,分析手术对靶症状、特别是冲动攻击行为的改善。结果术后1～3年随访,有效率为87.7%,显效率为47.8%,无效率为12.3%。结论CT引导立体定向术多靶点联合毁损术治疗精神分裂症是一种较安全、有效的方法,但要合理选择适应证及手术方式。术后应配合药物及心理等综合治疗,以巩固疗效。

The therapy of intractable schizophrenia accompanied by impetus assault action with united multi-target derogation operation

Objective To investigate the best operative manner,clinical therapic effects,safety and indication of intractable schizophrenia treated with united multi-target derogation operation of stereo localization conducted by CT.Method Treating intractable schizophrenia with united multi-target derogation operation of stereo localization and following-up survey the patients,analysing the effects of operation on systemic symptom,improvement of impetus assault action and syndrome,according to CGPS criteria.Result The therapic effect after operation is as follows:87.7%(effective),47.8%(effective specially)and12.3%(failure)followed-up survey within1-2years.Conclusion The therapic method is a safe and effective operation for intractable schizophrenia,the keys to success are choosing the right indication syndrome and operation manner and accurate localiza-tion.The administration after operation is very important
