子宫内膜腺癌组织Ki-67蛋白和caspase-3表达及意义

摘    要:目的探讨增殖细胞核抗原(Ki-67)蛋白及门冬氨酸特异的胱氨酸蛋白酶-3(caspase-3)在子宫内膜腺癌组织中的表达及其与临床病理参数间的关系,进而阐明它们在子宫内膜腺癌发生发展中的作用及其之间可能存在的关系。方法应用免疫组化EnviSion法检测20例正常子宫内膜和70例子宫内膜腺癌组织中Ki-67蛋白及caspase-3的表达情况。结果Ki-67蛋白在子宫内膜腺癌组织中的阳性表达率明显高于正常子宫内膜组织(Hc=27.81,P<0.01);caspase-3在子宫内膜腺癌组织中的阳性表达率明显低于正常子宫内膜组织(uc=5.097,P<0.01);Ki-67蛋白表达与病人年龄及手术-临床分期无关(Hc=3.590、5.888,P>0.05),而与病理分级、子宫肌层浸润深度及淋巴结转移有关(Hc=8.504、4.778,P<0.05;uc=2.963,P<0.01);caspase-3表达与病人年龄、手术-临床分期、子宫肌层浸润深度及淋巴结转移均无关(Hc=4.371,uc=1.077~1.374,P均>0.05),而与病理分级有关(Hc=7.320,P<0.05);子宫内膜腺癌组织Ki-67的表达与caspase-3呈负相关(rs=-0.241,P<0.01)。结论Ki-67蛋白与子宫内膜腺癌侵袭和转移关系密切;Ki-67蛋白和caspase-3可能呈负反馈调节,共同参与子宫内膜腺癌的发生、发展以及细胞凋亡与增殖的病理过程。

Abstract:Objective To investigate the expression of Ki-67 protein and caspase-3 in endometrial adenocarcinoma and its relationship with the parameter of clinical pathology and to interpret their role in the genesis and development of endometrial adenocarcinoma. Methods The expression of Ki-67 protein in endometrial adenocarcinoma and caspase-3 was detected by immunohistochemical EnviSion method in 20 cases of normal endometrium and 70 of endometrial adenocarcinoma. Results The positive expression rate of Ki-67 protein in endometrial adenocarcinoma was significantly higher than that in normal endometrium(Hc=27.81,P<0.01);the positive expression of caspase-3 was markedly lower in endometrial adenocarcinoma than that in normal endometrium(Hc=27.81,P<0.01).There was no relationship between Ki-67 protein expression and patient's age and clinical stage(Hc=3.590,5.888;P>0.05),but it was related to pathological grade,myometrial invasion and lymph node metastasis (Hc=8.504,4.778;uc=2.963;P<0.05,0.01).The expression of caspase-3 did not correlate with patient's age,clinical stage,myometrial invasion,and lymph node metastasis,but it correlated with pathological grade(Hc=7.320,P<0.05).Expression of Ki-67 was correlated negatively with caspase-3(rs=-0.241,P<0.01). Conclusion The expression of Ki-67 is closely related to the infiltration and metastasis of endometrial adenocarcinoma,acting as a negative feedback regulation,Ki-67 and caspase-3 may be involved in common in the genesis and development of endometrial adenocarcinoma,and in the pathological process of cell apoptosis and proliferation.
