摘    要:目的:探讨应用AO微创内固定系统(less invasive stabilization system,LISS)治疗胫骨近端骨折的临床效果。方法:2005年6月至2008年6月间,应用LISS治疗胫骨近端骨折32例,41-A2骨折5例,41-A3骨折8例,41-C1骨折7例,41-C2骨折6例,41-C3骨折6例。其中9例开放骨折均为Ⅱ-Ⅲ级,有6例患者为多发骨折患者,伤后4d(2-7d)应用LISS行内固定治疗。结果:所有患者随访6-18个月,骨折均连接,平均骨折愈合时间12周(8-18周),未出现钢板螺钉松动移位、骨折塌陷、复位丢失等。胫骨近端骨折32例,HSS评分优20例,良14例,可2例。膝关节活动范围90°-140°,所有患者膝关节行走无疼痛。结论:LISS能有效治疗各种分型的胫骨近端骨折,特别是对于骨质疏松患者具有更好的优越性。

Treatment of Proximal Tibia Fractures with Less Invasive Stabilization System

Abstract:Objective: To explore the clinical effect of less invasive stabilization system(LISS) for treatment of proximal tibia fractures. Methods: From June 2005 to June 2008, LISS was used for 32 cases of proximal tibia fractures (according to the AO classification, type 41-A2 5 cases, type 41-A3 8 cases, type 41-C1 7 cases, type 41- C2 6 cases, type 41-C3 6 cases). Among them, 9 patients with open fracture grade Ⅱ-Ⅲ 6 patients with multiple fractures. LISS fixation was performed at 4 days(2-7 days) after injury. Results: All the patients were followed up for 6-18 months,all the fracture were bony union after 12 weeks (8-18 weeks). There was no screws or plates loose or reduction loss happened. HSS score was evallnated as excellent in 20 cases, good in 14 cases and fair in 2 cases. The range of movement of knee joint was 90°-140°, and no pain was noted. Conclusion: LISS can be effectively used to treat all types of proximal tibia fractures, especially for the osteoporosis patients.
