苫参素注射液治疗慢性乙型肝炎与中医证型关系

目的：观察苦参素注射液治疗慢性乙型肝炎与中医征型的关系。方法：选择临床确诊为慢性乙型肝炎、肝功能损害、病毒呈高复制状态的80例门诊患者，中医辨征为肝胆湿热26例、肝郁脾虚32例、肝肾阴虚l3例、脾肾阳虚4例、瘀血阻络5例，给予苦参素注射液400mg或600mg，每日1次，肌注，疗程90d。结果：全部病例总有效率为75％，其中肝胆湿热型与肝郁脾虚型总有效率分别为88．5％和81．3％，明显高于其他各型(P＜0．01)；肝功能恢复及HBeAg,HBV-DNA阴转率也明显高于其他各型。结论：苦参素注射液治疗慢性乙型肝炎中医辨证属肝胆湿热型与肝郁脾虚型疗效较好，其中尤以肝胆湿热型为优。

The Relationship between Osymatrine Injection on Chronic Hepatitis B and TCM Type of Syndromes

Objective : To observe the relationship between Oxymatrine injection on chronic hepatitis B and TCM type of syndromes. Methods : 80 out patients were diagnosed to be chronic hepatitis B, liver function lesion and highly virtus reproducing TCM differentiation of syndromes and signs. Damp-heat of liver and gall 26; stagnation of the liver-qi and deficiency of the spleen 32; deficiency of liver-yin and kidney-yin 13; insufficiency of both the spleen and the kidney 4 ; blood stasis vessel obstruction syndrome 5. All above were given Oxymatrine injection, 400mg or 600mg, Qdx90days. Results: The total efficient rate was 75%, while the rate of damp-heat of liver and gall and stagnation of the liver-qi and deficiency of the spleen was 88.5% and 81.3% respectively, which were significantly higher than those of other types of syndromes (P<0.01). Normal lied liver function and negative rate of HBsAg, HBV-DAN were also significantly higher. Conclusion : Oxymatrine injection is effective in treating damp-heat of liver and gall and stagnation of the liver-qi and deficiency of the spleen of chronic hepatitis B especially in damp-heat of liver and gall treatment.
