锁定加压钢板与人工股骨头置换治疗老年不稳定型股骨粗隆间骨折的疗效比较

目的探讨锁定加压钢板与人工股骨头置换治疗老年不稳定型股骨粗隆间骨折的疗效。方法将2007年1月—2012年1月在我科手术治疗的82例老年不稳定型股骨粗隆间骨折患者分为两组,40例采用锁定加压钢板内固定,42例采用人工股骨头置换术,比较两种方法术中出血量、手术时间、卧床时间、再手术率及髋关节功能。结果两组在手术时间、术中出血量、术后卧床时间等比较,差别均有统计学意义(P<0.01);两组术后髋关节功能比较,差别无统计学意义(P>0.05)。结论在严格掌握手术适应证的情况下,锁定加压钢板内固定与人工股骨头置换均是治疗老年不稳定型股骨粗隆间骨折的有效的方法。

Therapeutic effect comparison of artificial thigh bone replacement and locking compression plate fixation for unstable intertrochanteric femoral fracture in elderly patients

Objective To analyze the therapeutic effect of locking compression plate fixation and artificial thigh bone replacement in treatment of unstable intertrochanteric femoral fracture. Methods 82 elderly patients with unstable intertrochanteric femoral fracture who were admitted to our department from January 2007 to January 2012 were divided into two groups.40 cases were treated with locking compression plate fixation,42 were treated with artificial thigh bone replacement.To compare the two treated groups in terms of the operative time,blood loss,bedridden time,reoperation rate and recovery of hip joint function. Results There were significantly differences in the operation time,blood loss and bedridden time(P<0.01);there was no significant difference in the functional recovery of hip joint function between the two groups(P>0.05). Conclusion Locking compression plate fixation and artificial thigh bone replacement are reasonable treatment for intertrochanteric femoral fracture in elderly patients and the surgical indication should be strictly mastered.
