超声刀在近端胃癌根治术中的应用分析

目的观察超声刀在近端胃癌根治术中的临床应用效果。方法将62例行经腹近端胃癌根治术的胃上部癌患者,随机分为超声刀组和高频电刀组各31例。比较两组患者的临床资料。结果超声刀组手术时间显著短于高频电刀组(P<0.01),出血量少于高频电刀组(P<0.05),两组间切口长度、放置引流管人数和淋巴结清除个数等指标相比无统计学差异(P>0.05)。超声刀组术后72h腹腔引流量显著少于高频电刀组(P<0.01),两组间术后镇痛时间、首次排气时间、术后住院时间、并发症等指标相比无统计学差异(P>0.05)。两组间术后生存曲线相比无统计学差异(P>0.05)。结论超声刀是施行经腹入路近端胃癌根治术时的理想工具。

Analysis on the application of ultrasonically activated scalpel in radical surgery for proximal gastric cancer

Objective To observe the clinical results in radical surgery for proximal gastric cancer using ultrasonically activated scalpel. Methods The 62 cases of proximal gastric cancer with radical surgery were randomly divided into ultrasonically activated scalpel group and high frequency electric scalpel group,each 31 cases.The clinical data were recorded and compared between two groups. Results The mean operating time in ultrasonically activated scalpel group shorter than those in high frequency electric scalpel group(P>0.01);the blood loss volume was significantly lower than those in high frequency electric scalpel group(P<0.05) and the drainage volume of first 72 hours was significantly lower than those in high frequency electric scalpel group(P>0.05).But the length of incision,number of placing drainage tube,number of lymph node retrieved,duration of postoperative analgesia,return of bowel motility,postoperative hospital stay,complications and survival curve had no significant difference between the two groups(P>0.05). Conclusion The ultrasonically activated scalpel is an ideal tool in radical surgery for proximal gastric cancer.
