肝脏酶谱检测在诊断肝病中的应用及其临床意义

目的探讨肝病患者血清中肝脏酶谱酶活性单位变化的临床特点及其对诊断该病的临床意义。方法选择2007年1月至2010年12月收治的200例肝病患者为观察组，另外选取同期84例健康体检者为对照组，观察比较两组样本血清中天门冬氨酸氨基转移酶（AST）、丙氨酸氨基转移酶（ALT）、碱性磷酸酶（ALP）、1-谷氨酰氨基转移酶（y—GT）和5’-核苷酸酶（5’-NT）的水平表达及其与肝病之间的关系。结果观察组各种肝病患者血清中的这5种酶均高于对照组，其中急性肝炎、肝硬化、原发性肝癌以及部分其他肝病患者的这5种酶活性单位均显著高于对照组（P〈0．05），慢性肝炎和酒精性肝炎略高于对照组。结论肝病患者血清中肝脏酶谱的表达水平与其肝病类型有一定的相关性，可以根据血清中肝脏酶谱水平的变化来判断患者是否有肝功能损害。

Study on significance of liver enzyme spectrum in diagnosis of liver diseases.

Objective To explore the characteristics and significance of＂ serum level of liver enzyme spectrum in diagnosis of patients with liver diseases and its relationship with diagnosis of liver diseases. Methods Two hundred patients with liver diseases in this hospital during Janu- ary 2007 to December 2010 were allocated in observation group; and 84 healthy persons were selected as control group. The difference in serum levels of liver enzymes as AST, ALT, ALP, ~/- GT, and 5＂- NT were compared between these 2 groups. Results The serum levels of AST, ALT, ALP, ~ - GT, and 5＂- NT in patients of observation group were higher than those of healthy persons in control group. The serum levels of liver enzymes in patients with acute hepatitis, liver cirrhosis and primary liver cancer were significantly higher than those of control group （ P 〈 0. 05 ）. Conclusion Spectrum of expression levels of serum liver enzymes in patients with liver diseases had certain correlation with types of liver diseases, and patients with impaired liver function can be determined according to changes of serum levels of liver enzyme spectrum.
