121例病毒性心肌炎柯萨基B病毒中和抗体的检测

本文使用微量中和试验方法,测定了121例病毒性心肌炎患者血清中抗柯萨基B中和抗体水平,并与40名正常人及35名其它疾病患者进行了比较,结果表明,当抗柯萨基B中和抗体滴度≤1:64时,三组间抗体的检出率无显著差异;当中和抗体滴度为1:128时,病毒性心肌炎组抗体检出率为61.98%(75/121),其它疾病组与正常人群组分别为13.51%(5/35)和12.5%(5/40)(P<0.001);当中和抗体滴度为1:256,1:512和1:1024时,病毒性心肌炎组抗体检出率分别为33.07%(40/121)。15.7%(19/121)和33.88%(41/121),而其它两组检出率皆为0。本文认为病毒性心肌炎患者抗柯萨练B中和抗体超过正常人群最高抗体水平者可以建立病原学诊断,其与心肌活检的病理变化相符,并与NK细胞活性的变化相一致。

Detection of Specific Anti-Coxsackie Group B Virus Neutralizing Antibodies in 121 Cases of Viral Myocarditis

Using the microtechnique of neutralization test, the authors detected specific anti-Coxsackie group B virus (CVB) neutralizing antibodies in 121 cases of viral myocarditis and compared the findings with 40 normal persons and 35 patients of other diseases. When the titre of anti-CVB antibody was 1:64, the detection rate among the three groups did not differ significantly. When the titer was 1:128, the detection rate was 61.98%(75/121) in the group with viral myocardial, 13.51% (5/35) in normal persons and 12.5%(5/40) in the non-myocardial group(P<0.001). When the titer was 1:256, 1:1024, the detection rate in the group with vital myocarditis was 33.70%(40/121),15.7%(19/121) and 33.88%(41/121)respectively, but was negative in the other two groups. We suggest that when the titer of neutralizing antiboty in patients with viral myocarditis is higher than the highest titer of normal persons, etiological diagnosis can be established, as it was proved by the changes in myocardial biopsy and the activity of natural killer cells.
