189例恶性淋巴瘤病理类型分析

目的分析恶性淋巴瘤病理类型和部位分布特征。方法收集本院病理科1993～2001年间诊断的淋巴瘤病例189例，应用WHO新分类进行分类。结果189例患者中，霍奇金淋巴瘤11例(5．8％)，非霍奇金淋巴瘤(NHL)178例(94．2％)。免疫表型显示，B细胞淋巴瘤160例(89．9％)，T细胞淋巴瘤18例(10．1％)。以弥漫大B细胞淋巴瘤(DLBCL)为主要病理类型117例(65．7％)，但其组织细胞学呈明显异质性。胃部约有1／3的病例为MALT型边缘区B细胞淋巴瘤(粘膜相关淋巴瘤)，未见T细胞淋巴瘤；而肠道可见到25．0％的病例为T细胞淋巴瘤；鼻腔鼻窦则以T细胞淋巴瘤为多数。结论本地区霍奇金淋巴瘤少见；结外NHL多于淋巴结，且B细胞淋巴瘤占绝大多数；DLBCL为最常见的病理类型，其它类型有明显的部位分布特征。

Analysis on pathological subtypes of 189 cases of malignant lymphoma

Objective To analyze the features of pathological subtypes and the site distribution of malignant lym phoma. Methods One hundred and eighty-nine lymphoma cases with clinical data, pathological and immunohisto chemical materials were collected from our department, from 1993 to 2001, These cases were classified according to the new WHO proposal Results Among 189 cases, there were 11 Hodgkin lymphomas (5.8% ) and 178 non-Hodgkin lymphomas (NHL, 94.2% ). Immunohistochemically, there were 160 cases of B cell lymphoma (89.9% ) and 18 cases of T cell lymphoma (10. 1% ). Diffuse large B cell lymphoma was the most common subtype in both lymph node and extra-node, types occuping 117 cases (65.7% ). But its histology was heterogeneous. About one third of gastric lymphomas were MALT-type marginal zone B cell lymphomas. NO gastric T cell lymphoma was found. One fourth of intestinal lymphoma were T cell lymphomas. Majority of nasal lymphomas were of T cell origin. Conclusion Hodgkin lymphoma is rare in this area. Extra-node NHLs are more common, and majority of NHL are of B cell origin. DLBCL is the most common subtype and the other subtypes show a prominent feature in site distribution.
