原发性脾脏淋巴瘤6例报告并文献复习

目的：探讨原发性脾脏恶性淋巴瘤(primary malignant lymphoma of the spleen，PLS)的病理形态学特征及诊断要点。方法：采用常规HE染色及免疫组化染色，对6例PLS进行观察。结果：粟粒状肿块2例，单发性肿块2例，多发结节状肿块2例，其形态特点与其组织学类型密切对应。镜下肿瘤由异型淋巴细胞构成，小细胞性2例，大细胞性和混合细胞性4例，大细胞性淋巴瘤相对多见。免疫标记显示多数病例(5／6)为B细胞源性。结论：原发性脾脏恶性淋巴瘤组织学形态复杂多变，多为B细胞源性，结合相应的形态学特征及免疫组化结果可确诊。

Primary Lymphoma of the Spleen:a Report of 6 Cases and Literature Review

Object: To explore the histopathological characteristics of splenic primary lymphoma and the criteria for its pathological diagnosis. Methods: Six cases were studied by HE-strained, immunohistochemitry, and review of the literature. Results: The gross patterns of the 6 cases includes miliary masses(2 cases), mutiple masses(2 cases), solitary masses(2 cases), which correspond closely to the various microscopic types. Large lymphocyte lymphoma(4 cases) is more often than small lymphocyte lymphoma(2 cases). In our study most of the cases(5/6) were originated from B cell.Conclusion: Histopathological changes of lymphoma primary occurred in the spleen were often variable. It's histopathological characteristics and immunophenotype are effective for the diagnosis of PLS.
