肺癌规范性手术及术中组织间放疗

在肺癌规范性手术中，有三点关键应予考虑：（１）肺的切除范围有多大？（２）如何处理文气管残端？（３）如何清除纵隔淋巴结？在左肺上叶癌中，应清除第１１、１０、７、６、５和２组的纵隔淋巴结，在右肺上叶癌中，应清除第１１、１０、７、４和３组的纵隔淋巴结，下叶和中叶中应清除第１１、１０、９、８和７组淋巴结。根据上述规则，我们设计了“肺癌规范性手术”原则，并按此原则对１５４例肺癌患者进行了此项手术，在１期患者与传统手术对比，规范性手术显著地改善了疗效。

Standardized Operation for Lung Cancer with Radiotherapy in Operation

There have three main points to beconsidered to the standardized operation for lung can-cer :1. How many regions of the lung should be re-sected? 2.How to manage the bronchial stump? 3.How to clean the mediastinal lymph nodes.In thelung cancer of upper left lobe,11,10,7,6, 5 and 2groups of mediastinal lymph nodes should be takenout.In the upper right lobe,11,10,7,4 and 3 groupsshould be taken out.And in the lower and mediallobe,11,10,9, 8 and 7 groups should be dissected.According to the above rules,we designed a principleof “Standardized Lobectomy for Lung Cancer”and154 cases of lung cancer were treated under this prin-ciple of operation.The resluts of standardized operation in thestage Ⅰcases are markedly improved in comparisonto that of the conventional operation.
