胸腔镜胸腺切除术治疗重症肌无力的优势

摘    要:目的 评价电视胸腔镜下胸腺切除术治疗重症肌无力的优势.方法 回顾性研究42例经电视胸腔镜行胸腺切除术治疗的重症肌无力患者(胸腔镜组),与胸骨正中切口下胸腺切除术组(开胸组)42例比较,分析其手术时间、术后住院时间等临床指标.结果 2组患者均无手术死亡.胸腔镜组的手术时间为(90.6±7.7)min,开胸组为(101.7±8.9)min;胸腔镜组的术中出血为(136.9±45.7)ml,开胸组为(246.0±60.7)ml;胸腔镜组的术后住院时间为(8.2±2.7)d,开胸组为(11.1±5.4)d;胸腔镜组的胸管引流时间为(3.3±1.5)d,开胸组为(4.3±2.1)d.2组的手术时间、术中出血、术后住院时间差异有统计学意义(均P<0.01);术后胸管引流时间差异亦有统计学意义(P<0.05).结论 胸腔镜胸腺术治疗重症肌无力能减轻术中创伤,缩短术后恢复时间.对于合适的患者可作为首选术式.
Abstract:
Objective To examine the superiority of video-assisted thoracoscopic surgery in patients with myasthenia gravis (MG) compared to transstemal thymectomy. Methods The data of 42 patients with thymectomy by thoracoscopic approach were analyzed retrospectively and compared to the transstemal thymectomy group of 42 patients. Then we study the patient selection, operative techniques, perioperative management and results for thoracoscopic approach. Results The thoracoscopic group had a mean operating time of (90. 6 ±7. 7) minutes compared with(101.7 ±8.9) minutes in the transstemal group. The thoracoscopic group had a mean estimated blood loss of (136.9 ±45.7)ml compared with(246.0 ±60.7)ml in the transstemal group. The mean length of stay postopera-tively for the thoracoscopic group was(8.2 ±2.7)days and was(ll. 1 ±5.4)days in the transstemal group. The thoracoscopic group had a mean time of chest draining of ( 3. 3 ± 1. 5 ) days compared with ( 4. 3 ± 2. 1) days in the transstemal group. Both thoracoscopic and transstemal approaches to thymectomy in patients with MG are effective. The thoracoscopic group has the added benefits of decreased time of operation, decreased estimated blood loss, decreased length of stay ( P < 0. 01 ) , and decreased time of chest draining ( P < 0. 05 ) compared to the open group. Conclusion The thoracoscopic approach for thymectomy is safe and feasible fro the treatment of MG.
