经鼻内镜垂体瘤切除术中七氟烷或异氟烷麻醉效能比较研究

目的:比较七氟醚和异氟醚对鼻内镜下经鼻垂体瘤除术的麻醉效果及并发症。方法:选择ASAⅠ～Ⅱ级择期行鼻内镜下经鼻垂体瘤除术的患者41例机分为两组;七氟醚组(21例)和异氟醚组(20例)。两组诱导用药相同:咪达唑仑3～5mg,芬太尼4μg/kg,维库溴铵0.1mg/kg,依托咪酯0.3mg/kg。麻醉维持:持续泵入瑞芬太尼,吸入七氟醚维持麻醉为七氟醚组,吸入异氟醚维持麻醉为异氟醚组。观察指标:患者围术期个时点的血压、心率、血氧饱和度及术中术后并发症。结果:两组患者诱导后l组血压下降幅度大于S组,但差异无统计学意义(P>o.05)。两组间在同一时间点的血压比较亦无显著性差异。两组患者心率(HR).组间及组内比较,均无显著性差异。睁眼时间七氟醚组为(20.0±13.9)min,异氟醚组为(21.6±11.7)min。两组均无术中知晓发生,亦未见其他麻醉并发症。结论:七氟醚用于短时间手术的麻醉,苏醒期躁动、恶心呕吐的发生情况明显低于以往的文献报道。虽然七氟醚用于神经外科手术的麻醉存在争议,本实验结果显示,用于神经外科短时间手术的麻醉安全可靠,又能保证血流动力学的稳定使患者快速苏醒。

The Nasal Endoscopic Pituitary Tumor Resection of Seven Fluorine or the Daughter Anesthesia Fluorine Comparative Study Effi ciency

Objective:Tto compare me sevoflurane and isoflurane nasal endoscopic nasal pituitary tumor enucleation anaesthetic effect and complications.Methods:ChooseⅠ～Ⅱelective class ASA did the nasal endoscopic pituitary adenoma of 41 cases,patients were randomly divided into two groups:sevoflurane group(21 cases) and isoflurane group(20 cases).Two groups of induction drug use the same:mi) of hebron 3～5mg,fentanyl 4μg/kg,d library 0.1 nig bromine ammonium/kg.relying on 0.3mg) cster/kg.Anesthesia maintain:continuous pump into the red fentanyl,inhaled sevoflurane anesthesia for maintaining sevoflurane group,inhaled isoflurane maintain anesthesia for isoflurane group.Outcome:perioperative period in patients with a point of blcod pressure and heart rate,blood oxygen saturation and operative complications.Results:Two groups of patients after inductionⅠdrop in blood pressure group was more group S,but difference was statistically significant(P>0.05).Between the two groups in the same time points compared blood pressure to have no significant difference.Two groups of patients heart rate(HR),and between groups in the group is,there are no significant difference.Open your eyes time sevoflurane group for(20.0±13.9)min,isoflurane group for(21.6±11.7)min.Both groups not to have art that happened in,also has not seen the other anesthesia complications.Conclusion:Sevoflurane anesthesia for a short time to wake up the operation period,nausea and vomiting restless happened obviously lower than the previous titerature.Although sevoflurane anesthesia for nerve surgery debate,the experimental results show that,for neurosurgery short time of the surgery anesthesia and guarantee the safe and rehable,stable and rapid hemodynamic waking up.
