GEDVI在脓毒症休克中的应用价值

目的:探讨全心舒张末期容量指数(GEDVI)在脓毒症休克中的应用价值。方法:对我院收治的21例脓毒症休克的病人在感染性休克确认后立即留置动脉轮廓法(PICCO)导管,进行PICCO监测,每6个小时通过注冰盐水测全心舒张未容积(GEDV)及GEDVI,每博量(SV)及指数(SI),并同时通过中心静脉导管进行了中心静脉压(CVP)的监测,记录每次监测的数据。结果:经过线性回归分析,GEDVI的值与SI的值有较好的相关性,r=0.735,CVP与SI的相关性较差,r=0.546;△GEDVI与△SI的相关性为r=0.779;△CVP与△SI的相关性为r=0.258。结论:GEDVI更能反映病人的前负荷,容量的指标比压力的指标更能反映病人的前负荷,以GEDVI指导早期液体复苏治疗(EGDT)的更有效。

GEDVI in Sepsis Shock Value

Objective To explore the application value of GEDVI in sepsis shock.Methods:The 21 septic shock patients of hospital treated in septic shock after confirming the indwelling catheter immediately PICCO,PICCO monitoring,injected iced saline measuring heart diastolic volume(GEDV ) and no index(GEDVI),stroke volume(SV ) and index(SI) every 6 hours,and recorded each monitoring data of central venous pressure(CVP ) by central venous catheter at the same time..Results:Through linear regression analysis,GEDVI value and SI value had good correlation,r = 0.735,CVP and SI correlated were poorer,r = 0.546,;Delta GEDVI and delta SI correlation were r = 0.779:CVP and SI correlation were r = 0 258 Conclusion:GEDVI could reflect the patient’s previous load,capacity indicator reflect the patient’s previous load were better than the pressure index,guidance of early fluid resuscitation(EGDT ) were more effective by GEDVI.
