扩张后胸三角皮瓣带蒂转移修复术治疗面颈部烧伤后瘢痕的临床效果

目的 分析面颈部烧伤后瘢痕采用扩张后胸三角皮瓣带蒂转移修复术对术后修复组织挛缩率与疼痛程度的影响。方法 选取武警河北总队医院与河北医科大学第一医院2019年2月-2023年6月收治的60例面颈部烧伤后瘢痕患者为研究对象，采用随机数字表法分为对照组和观察组，各30例。对照组予以游离皮片移植手术治疗，观察组予以扩张后胸三角皮瓣带蒂转移修复术治疗，比较两组修复组织挛缩率、修复效果、并发症发生情况及疼痛程度。结果 观察组术后1、6个月修复组织挛缩率低于对照组（P ＜0.05）；观察组修复后组织表面平整、皮肤色泽、感觉恢复、弹性质地、整体美观感评分均高于对照组（P ＜0.05）；观察组并发症发生率低于对照组（P＜0.05）；观察组术后1、6个月VAS评分低于对照组（P＜0.05）。结论扩张后胸三角皮瓣带蒂转移修复术用于面颈部烧伤后瘢痕治疗中能够改善修复效果，降低术后疼痛程度及并发症发生几率，降低术后修复组织挛缩发生几率，在临床中应用价值较高。

Clinical Effect of Expanded Deltopectoral Flap Pedicled Transfer Repair in the Treatmentof Scar After Facial and Neck Burn

Objective To analyze the effect of expanded deltopectoral flap pedicled transfer repair on postoperative tissue contracturerate and pain degree after facial and neck burn scar. Methods From February 2019 to June 2023, 60 patients with scars after facialand neck burns admitted to Hebei General Hospital and the First Hospital of Hebei Medical University were selected as the researchobjects. They were divided into control group and observation group by random number table method, with 30 patients in eachgroup. The control group was treated with free skin graft, and the observation group was treated with pedicled transfer repair ofexpanded deltopectoral flap. The repair tissue contracture rate, repair effect, complications and pain degree were compared betweenthe two groups. Results The repair tissue contracture rate of the observation group was lower than that of the control group at 1and 6 months after operation (P <0.05). The scores of tissue surface leveling, skin color, sensory recovery, elastic texture and overallaesthetic feeling in the observation group were higher than those in the control group (P <0.05). The incidence of complicationsin the observation group was lower than that in the control group (P <0.05). The VAS score of the observation group was lowerthan that of the control group at 1 and 6 months after operation (P <0.05). Conclusion The application of expanded deltopectoralflap pedicled transfer repair in the treatment of scars after facial and neck burns can improve the repair effect, reduce the degree ofpostoperative pain and the incidence of complications, and reduce the incidence of postoperative repair tissue contracture. It has high clinical application value.
