高危儿脑损伤的早期干预方法及效果研究

摘    要:目的研究高危儿脑损伤的早期干预方法及效果。方法采取病例对照研究方法,并用《Gesell发育诊断量表》进行效果评估。结果干预组195例脑损伤儿经过早期医学康复干预后,186例(95.39%)恢复了正常,临界水平8例(4.10%),异常1例(0.51%)。3组小儿1岁时Gesell智能检查结果比较,干预组患儿发育水平明显高于未干预组,5个能区发育商有极显著性差异(P<0.01);干预组与对照组比较,5个能区差异无显著性(P>0.05);未干预组与对照组比较,未干预组各能区发育极显著落后于对照组(P<0.01)。结论早期综合康复干预能促进脑损伤儿的运动、智能发育,有效降低脑瘫、智力低下的发生。

Abstract:Objective To study the method and the effectiveness of early intervention measures on high-risk neonates with brain damages.Methods The efficacy of early intervention measures on high-risk neonates with brain damages were evaluated with Gesell Development-diagnosis Scale in this case-controlled study.Results In the intervened group,186 cases(95.39%) resumed normal,critical level 8 cases(4.10%),and abnormal(cerebral palsy) 1 case(0.51%).The neonates is these 3 groups reevaluated by Gesell intelligence assessment at the age of 1 year showed that the intelligence development level in the intervened group was significantly higher than that of the non-intervened group,and there were statistically significant differences in the development quotients of adaptive behavior,gross motor,fine motor,language,and personal-social behavior(P<0.01).The development quotients of adaptive behavior,gross motor,fine motor,language,and personal-social behavior showed no statistically significant difference between the intervened group and the control group(P>0.05).The development quotients of adaptive behavior,gross motor,fine motor,language,and personal-social behavior in the non-intervened group were significantly lower than those of the control group(P<0.01). Conclusion Early comprehensive recovery intervention can promote both motor and mental development on brain damage neonates and reduce the occurrence of cerebral paralysis and amentia.
