硫糖铝山莨菪碱防治重型脑伤应激性溃疡的疗效

目的探讨硫糖铝、山莨菪碱混悬液早期鼻饲，防治重型脑伤应激性溃疡的疗效。方法回顾性对比分析903例重型脑损伤患者（对照组：380例，伤后72h内无任何预防应激性溃疡措施。A组：303例，使用鼻饲雷尼替丁、甲氰咪胍、奥美拉唑；B组：220例，早期鼻饲硫糖铝和山莨菪碱混悬液）应激性溃疡出血、肺部感染发生情况。结果B组应激性溃疡的发生率为1．6％，肺部感染发生率为1．3％，与A组和对照组比较均明显降低（P〈0．05），输血治疗数和病死率同步下降。结论在治疗原发脑伤的同时，硫糖铝、山莨菪碱混悬液早期鼻饲对防治重型脑伤应激性溃疡具有重要临床价值，并能减少肺部感染发生率。

Comparison study on effects of sucralfate suspension and the anisodamine treatment of the irritability ulcer of the heavy brain damage

Objective To study the effects of sucralfate suspension and the anisodamine treatment of the irritability ulcer of the heavy brain damage. Methods Compared and analyzed the irritable ulcer bleeding and the lung infection in 903 heavy brain damage patients （Comparing Group： 380 patients, none treatment was done after 72 hours of injured. Group A： 303 patients used ranitidine, cemitidine and omeprazole as treatment. Group B： 220 patients used the mixture suspension of sucralfate and anisodamine through nasogastric feeding）. Results The rate of the irritability ulcer in Group B was 1.6% , and the rate of the lung infection was 1.3% which were smaller compared with Group A and the Comparing Group （ P 〈 0. 05 ）. Apart from this, the percentage of blood transfusion treatment and the death rate were all decreased. Conclusions Sucralfate suspension and the anisodamine treatment of the irritability ulcer of the heavy brain damage can reduce the rate of the lung infection.
