超声引导下无水酒精硬化治疗先天性肝囊肿68例疗效分析

目的探讨超声引导下无水酒精硬化治疗先天性肝囊肿的疗效。方法回顾性分析协和医院2002年1月～2007年1月68例超声引导下无水酒精硬化治疗的先天性肝囊肿患者的临床资料,比较不同直径的囊肿的疗效差异。结果82个囊肿治疗后随访10～69个月,平均29.8个月,治愈率64.6%,总有效率97.5%。其中直径〈8cm的囊肿治愈率为79.6%,直径≥8cm的囊肿治愈率为42.4%,两者差异有极显著性意义（P〈0.01）。除部分患者有明显疼痛、醉酒感外,无其它并发症。结论无水酒精硬化治疗先天性肝囊肿方法简便、创伤小、并发症少、疗效好、重复性好,患者耐受性好,是治疗先天性肝囊肿的理想方法。其中囊肿直径较小者治愈率较高,建议先天性肝囊肿患者有治疗指征时及早治疗。

Therapeutic effects of ultrasound-guided absolute ethanol sclerotherapy for 68 cases of congenital hepatic cysts

Objective To explore the therapeutic effect of ultrasound-guided ethanol sclerotherapy for congenital hepatic cysts. Methods The clinical data of 68 cases of congenital hepatic cysts treated in Union Hospital by absolute ethanol sclerotherapy from 2002 to 2007 were retrospectively analyzed, the therapeutic effects were observed, and the therapeutic effects in groups of different diameter of cysts were compared. Results During a follow-up period of 10-69 months （mean 29.8 months） after treatment, the curative rate was 64.6%, and the total effective rate was 97.5%. The curative rate for cyst of diameter 8 cm was 79.6%, ≥ 8 cm was 42.4%, and there was significant difference between the two groups （P〈0.01）. Except pain and intoxication, there was no other complication. Conclusion Absolute ethanol sclerotherapy is a simple and effective treatment with minimal trauma and less complications for congenital hepatic cysts. The curate rate is higher if the cyst is smaller. It is recommended that the patients with congenital hepatic cysts should be treated timely.
