连续性血液滤过联合乌司他丁治疗全身炎症反应综合征患者临床研究

目的:探讨联合应用连续性血液滤过及乌司他丁治疗全身炎症反应综合征患者的临床疗效及安全性。方法:将符合诊断标准的84例患者随机分为治疗组A(乌司他丁联合血液滤过)、治疗组B(血液滤过)及对照组各28例,观察治疗前、后患者炎症参数、呼吸及血液动力学等指标,抽取外周血测定血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)的浓度,并记录住重症监护病房(ICU)时间、总住院时间、死亡率等。结果:联合应用连续性血液滤过及乌司他丁可显著改善全身炎症反应综合征患者临床症状及检测指标,降低血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平,缩短住院时间,降低死亡率,结果均优于对照组(P<0.05)。结论:联合应用连续性血液滤过及乌司他丁可显著改善全身炎症反应综合征患者的临床症状,明显提高SIRS患者的疗效,显著降低死亡率。

Clinical Research of Systemic Inflammatory Response Syndrome in Treatment by Combining Continuous hemofiltration and Ulinastatin

Objective：To evaluate the clinical efficacy and safety in treatment systemic inflammatory response syndrome by combining continuous blood filtration and ulinastatin.Methods：Eighty-four cases with SIRS patients were randomly divided into treatment group A（treated with ulinastatin and hemofiltration）,treatment group B（treated with hemofiltration）and control group,28 cases of each group.The patients inflammatory parameters,respiratory and hemodynamic indices,and lab exams such as IL-6,TNF-α were detected before and after treatment,and record live ICU time,total length of hospital stay,mortality,etc.Results：The combination of continuous hemofiltration and ulinastatin can significantly improve SIRS patient＇s clinical symptoms and test indicators,and reduce IL-6 and TNF-α level,shorten the length of time,reduce mortality,the results were better than the control group（P0.05）.Conclusion：Ulinastatin and continuous hemofiltration combination therapy can improve the clinical symptoms,improve the curative effect of SIRS,significantly reduce mortality.
