抗环瓜氨酸肽抗体、抗角蛋白抗体与类风湿因子联合检测类风湿关节炎的诊断价值

目的:研究抗环瓜氨酸肽(抗CCP抗体)、抗角蛋白抗体(抗AKA抗体)和类风湿因子(RF)的联合检测在类风湿关节炎(RA)诊疗中的临床价值。方法:选取2009年1月—2010年11月我院门诊及住院患者210例,其中RA组110例,其他风湿性疾病组65例,健康对照组35例,所有患者均检测抗CCP抗体、抗AKA抗体和RF,分别用酶联免疫吸附试验(ELISA)、间接免疫荧光法、乳胶凝集法检测。结果:抗CCP抗体、抗AKA抗体和RF对RA诊断的灵敏度分别为72.7%、42.7%、82.7%,抗CCP抗体与RF灵敏度之间差异无显著性,RF及抗CCP抗体均明显高于抗AKA抗体;3种抗体对RA诊断的特异度分别为95%、99%、77%,抗CCP抗体、抗AKA抗体的特异度均比RF高。对RA的诊断联合3种抗体均阳性特异性为99%,灵敏度为40%;3种抗体中的任2项阳性的特异度为100%,灵敏度为33.6%;1种抗体阳性的特异度为54%,灵敏度为26.4%,3种抗体均阳性与1种抗体阳性的灵敏度及特异度的差异均具有统计学意义。结论:在目前应用的3种特异性抗体中,抗CCP抗体对RA的诊断价值最高,联合检测抗CCP抗体、抗AKA抗体和RF可以提高RA的诊断价值。
