甲亢性心脏病心功能不全危险因素分析

目的通过对甲亢性心脏病心功能不全患者的临床资料进行统计学分析,探讨其发生的危险因素。方法通过对295例甲亢,其中用128例甲亢性心脏病的心功能不全作为因变量,把性别、年龄、病程、常见的甲亢病因及FT3、FT4、S-TSH等多个因素进行多个自变量的Logistic逐步回归分析。结果发现患毒性弥漫性甲状腺肿的比其他危险因素出现心功能不全的危险性高2.193倍,病程增加的危险性高1.075倍,年龄增加的危险性高1.069倍,FT4增加的危险性高1.044倍。结论毒性弥漫性甲状腺肿、病程长、年龄大、FT4高浓度是甲亢患者出现心功能不全的独立危险因素,及时针对性治疗,对预防甲亢患者心功能不全有益。

Analysis of Risk Factors of Cardiac Insufficiency in patients with Hyperthyroid Heart Disease

Objective To explore the risk factors of cardiac insufficiency in the patients with hyperthyroid heart diseases . Methods Of 295 patients with hypetthyroidism,128 suffered from heart disease accompanied with cardiac insufficiency and 167 did not.The sex,age,course of disease, FT3, FT4, S-TSH and etiological factors were analysed by multiple independent variables stepwise logistic regression in all the patients. Results The factors related to risk of cardiac insufficiency in the patients with hyperthyroid heart disease included Graves' disease (OR, 2.193), course of disease(OR, 1.075), age (OR, 1.069)and FT4 (OR, 1.044). Conclusions Graves' disease ,the longer course or disease ,great age and higher FT4 were the independent risk factors of cardiac insufficiency. The curative measures directed against the above-mentioned risk factors will be beneficial to prevention and curing of cardiac insufficiency in the patientss with hypetthyroidism.
