胸腔镜辅助小切口病灶清除术治疗胸壁结核的效果

摘    要:目的:探讨胸腔镜辅助小切口病灶清除术治疗胸壁结核的效果。方法:回顾性分析2015年1月至2018年12月该院收治的62例胸壁结核患者的临床资料,根据手术方案不同分为对照组与观察组,各31例。对照组患者采用传统胸壁结核病灶清除术治疗,观察组患者给予胸腔镜辅助小切口胸壁结核病灶清除术治疗。比较两组手术相关指标水平、术后并发症发生率及术后1年复发率。结果:观察组患者手术时间长于对照组,术中出血量、术后引流量少于对照组,切口愈合时间、住院时间短于对照组,差异均有统计学意义(P<0.05);观察组并发症发生率为3.23%,低于对照组的45.16%,差异有统计学意义(P<0.05);观察组术后1年复发率为0,低于对照组的19.35%,差异有统计学意义(P<0.05)。结论:胸腔镜辅助小切口胸壁结核病灶清除术治疗胸壁结核患者效果显著,能减轻创伤,降低并发症发生率,促进患者康复,且术后复发率较低,效果优于常规手术。

Abstract:Objective: To analyze the clinical effects of thoracoscopic-assisted small incision lesion removal in treatment of chest wall tuberculosis. Methods: The clinical data of 62 patients with chest wall tuberculosis admitted to the hospital from January 2015 to December 2018 were retrospectively analyzed. According to the surgical plans, they were divided into control group and observation group, each with 31 cases. The control group was treated with traditional chest wall tuberculosis lesion removal, while the observation group was treated with thoracoscopicassisted small incision lesion removal. The levels of surgery-related indicators, the incidence of postoperative complications and the recurrence rate 1 year after the surgery were compared between the two groups. Results: The operation time of the observation group was longer than that of the control group;the amount of blood loss and the postoperative drainage volume were less than those of the control group;the incision healing time and hospital stay were shorter than those of the control group;and the differences were statistically significant(P<0.05). The complication rate in the observation group was 3.23%, which was lower than 45.16% in the control group, and the difference was statistically significant(P<0.05). The 1-year recurrence rate in the observation group was 0, which was lower than 19.35% in the control group, and the difference was statistically significant(P<0.05). Conclusions: Thoracoscopic-assisted small incision lesion removal in the treatment of chest wall tuberculosis has a significant effect, which can decrease the trauma, reduce the incidence of complications, promote the patients’ recovery as soon as possible, and have a lower postoperative recurrence rate. Moreover, it is superior to traditional surgery.
