双腔支气管导管插管三种定位方法的比较

目的:比较肺部听诊法、听诊 吸痰管法及纤维支气管镜法判断双腔支气管插管定位的准确性。方法:300例需双腔支气管插管单肺通气行胸科手术的患者随机分为3组,每组100例,气管插管后分别采用听诊法、听诊 吸痰管法及纤维支气管镜法判断双腔支气管的位置。结果:听诊法、听诊 吸痰管法、纤维支气管镜法定位准确率分别为64%、81%、100%,纤维支气管镜法明显高于其它2种方法(P<0.05),听诊 吸痰管法高于听诊法(P<0.05)。结论:听诊 吸痰管法能提高准确率,纤维支气管镜定位双腔管插管更准确可靠。

Compared study for three methods to place double-lumen endobronchial tubes

Objective:To compare the correction of placement of double-lumen endobronchial tubes by using fibroptic bronchoscope,auscultation and suction catheter, auscultation. Methods: Three hundred patients requiring double-lumen endobronchial tubes insertion for thoracic surgery were prospectively studied.after intubation, positions of the tubes were judged by three methods respectively. Results: It was found that tubes were placed satisfactorily after intubation in 64% patients by auscultation, 81% by auscultation and sucker and 100% by fibroptic bronchoscopy. Conclusion: Fibroptic bronchoscopy is a reliable technique for correct position of the double-lumen endobronchial tubes and should be used routinely.
