有氧运动对乳腺癌化疗后癌因性疲乏患者血清促肾上腺皮质激素和皮质醇水平的影响

摘    要:目的探讨有氧运动对乳腺癌化疗后癌因性疲乏(CRF)患者血清促肾上腺皮质激素(ACTH)和皮质醇(Cor水平的影响。方法选取2017年1月至2019年6月我院门诊就诊的乳腺癌化疗后CRF患者88例,随机分为干预组和对照组,每组各44例。两组均予知识宣教、心理干预和饮食指导等基本治疗措施,干预组在此基础上加有氧运动治疗,两组均治疗12周。观察并比较两组治疗前与治疗12周后血清ACTH和Cor水平及CRF症状的变化。结果治疗12周后,两组血清Cor水平较治疗前明显上升,ACTH水平明显下降(P0.05或P0.01),且干预组变化幅度较对照组更明显(P0.05);两组各个维度Piper疲乏评分较前明显下降(P0.05或P0.01),且干预组下降幅度较对照组更明显(P0.05)。结论有氧运动用于乳腺癌化疗后CRF患者能更明显升高血清Cor水平,降低血清ACTH水平,调节下丘脑-垂体-肾上腺皮质轴功能紊乱。

Effects of aerobic exercise on serum adrenocorticotropic hormone and cortisol levels in patients with cancer-related fatigue after breast cancer chemotherapy

Abstract:Objective To explore the effect of aerobic exercise on the levels of serum adrenocorticotropic hormone(ACTH) and cortisol (Cor) in patients with cancer-related fatigue (CRF) after breast cancer chemotherapy.Methods A total of 88 patients with CRF after breast cancer chemotherapy in our hospital from January 2017 to June 2019 were randomly divided into intervention group and control group,with 44 cases in each group.The two groups were given basic treatments such as knowledge education,psychological intervention and dietary guidance.The intervention group added aerobic exercise therapy on the above basis.Both groups were treated for 12 weeks.The changes of serum ACTH and Cor levels and CRF symptoms before and after treatment between the two groups were observed and compared.Results After 12 weeks of treatment,the serum Cor levels of the two groups increased significantly compared with those before,and the ACTH levels decreased significantly (P<0.05 or P<0.01).And the change of the intervention group was more obvious than that of the control group (P<0.05).Piper fatigue score of each dimension of the two groups was significantly lower than that before (P<0.05 or P<0.01),and the intervention group decreased more significantly than the control group(P<0.05).Conclusion Aerobic exercise in patients with cancer-related fatigue after breast cancer chemotherapy can significantly increase serum Cor levels,reduce serum ACTH levels,and regulate hypothalamic-pituitary-adrenal cortex axis dysfunction.
