重度颅脑损伤预后高危因素分析

目的　探讨影响重型颅脑损伤患者预后的某些高危因素。方法　回顾性分析我院中心ICU 1997年元月～ 2 0 0 3年元月收治的 49例重型颅脑损伤病人的临床资料 ,观察早期血压、血糖、Glassgow评分等三个因素对重型颅脑损伤患者预后的影响。结果　早期低血压组病死率高于非低血压组 (P <0 .0 5 ) ,2 4h随机血糖≥ 11.1mmol/L组病死率高于 <11.1mmol/L组 (P <0 .0 5 ) ,GCS分值愈低 ,病死率愈高 (P <0 .0 1)。结论　早期血压、血糖、Glassgow评分均可作为判断重型颅脑损伤预后有价值的临床指标。

Analysis of risk prognostic factors for patients with severe craniocerebral injury

Objective To explore some risk prognostic factors for patients with severe craniocerebral injury.Methods The clinical data of 49 patients with severe craniocerebral injury in ICU from Jan 1997 to Jan 2003 were analyzed.Blood pressure,blood sugar,and Glassgow score in the prognosis of patients with severe craniocerebral injury were observed.Results The mortality rate of hypotension group was much higher than that without hypotension(P<0.05).The mortality rate of patients with FBS≥11.1mmol/L in first 24h were higher than that of patients with FBS<11.1mmol/L(P<0.05).Patients with lower Glassgow score had higher death rate(P<0.01).Conclusion Blood pressure,blood sugar and Glassgow score might be valuable prognostic predictor for patients with severe craniocerebral injury severe.
