58例妊娠合并糖尿病的临床观察

目的 探讨妊娠合并糖尿病对母儿所造成的影响以及治疗对妊娠结局的影响。方法 选择1994～2003年间妊娠合并糖尿病孕妇58例与同期65例正常孕妇进行比较；并根据血糖控制情况将55例妊娠合并糖尿病孕妇分为血糖控制满意组（A组），共40例；血糖控制不良组（B组）．共15例，进行比较。结果 与正常孕妇组比较，妊娠合并糖尿病孕妇并发症（妊娠高血压疾病、羊水过多、胎膜早破、早产及剖宫产）显著升高（P〈0．05），围产儿并发症（死胎、新生儿窒息、巨大儿及RDS和畸形）的发病率亦显著升高（P〈0．05）。B组妊娠高血压疾病、早产发生率和新生儿窒息率、死胎发生率均显著高于A组。结论 妊娠合并糖尿病对母儿造成严重危害，加强对其管理和合理治疗可明显减少孕产妇并发症及死亡率，改善母儿结局。

Clinical analysis of maternal and fetal outcome in 58 cases of pregnant women with diabetes

Objective To investigate the effects of pregnant women with diabetes to on maternal and fetal outcome. Methods We compared the 58 cases of pregnant women with diabetes from 1994 to 2003 with 65 cases of normal pregnancy, and we divided the 55 cases of pregnant women with diabetes into two groups. Group A with well-controlled glucose had 40 cases, and group B without well-controlled glucose had 15 eases. Results In this study, we found among the 58 cases of diabetes women, the morbidity of PIH, polyhydramnios, premature rupture of membrane, premature labor, fetal death, asphyxia of newborn, fetal macrosomia, RDS, and newborn malfornation were signifiant higher than those in normal pregnant women. We also found the rate of cesarean section in diabetes women was signifiant higher than that in normal pregnant women. The morbidity of PIH, premature labor, asphyxia of newborn and fetal death in group B was signifiant higher than that in group A. Conclusion Augmentating management of pregnant women with diabetes and ideally controlling blood glucose may reduce the maternal complications and periantal morbidity.
